Referring Someone to Oak Care Home
We welcome referrals from local authorities, social workers, care coordinators, NHS teams, probation services, and family members. Our referral process is straightforward, confidential, and designed to move quickly so that people get the support they need without unnecessary delay.

- NHS community mental health teams
- Probation and criminal justice services
- Homelessness and outreach organisations
- Family members or carers
- Self-referrals (with supporting information)
Who We Support
We provide supported accommodation for adults aged 18 and over who require a safe, stable home environment with structured support. We specialise in supporting people with:
- Mental health needs
- Learning disabilities
- History of homelessness or housing instability
- Substance misuse recovery
- Care leavers and young adults in transition
- Complex needs requiring person-centred support
Step by Step
Our Referral Process
Submit a Referral
Complete the referral form below or contact us directly by phone or email. Please include as much background information as possible to help us assess suitability.
Initial Assessment
Our team will review the referral within 2 working days and contact you to discuss the person's needs, current situation, and any risk factors.
Placement Discussion
If we have a suitable placement available, we will arrange a pre-placement visit and discuss the support plan with the referrer and, where appropriate, the individual.
Move-In & Support Plan
Once agreed, we coordinate a smooth move-in and put a personalised support plan in place from day one, reviewed regularly with the resident and their network.
